When a surgeon completes the Apply to Operate application form, we collect the details entered on that form, such as name, contact information, and professional background. When you email or call us, we collect the information you share in that message. The second is information collected automatically. Like most websites, our hosting provider records standard technical information such as your IP address, browser type, and the pages you view, which is used to operate and secure the site.
We use the information you provide to respond to your inquiry, to review and follow up on surgeon applications, and to communicate with you. We use automatically collected technical information to keep the website running properly and to protect it against misuse. We do not use your information for advertising, and we do not sell it.
We do not sell or rent your personal information. Application form submissions are processed by Formspree, a third-party form service that delivers your submission to us by email. Our website is maintained by our hosting provider. We may disclose information if required to do so by law or to protect our rights. Aside from these cases, we do not share your information with outside parties.
Our website does not use advertising, analytics, or tracking cookies, and we do not track you across other websites. To display the site, standard web fonts may be loaded, and when you submit the application form, your submission is transmitted to our third-party form provider so that it can reach us. If we introduce analytics or other cookies in the future, we will update this policy and, where required, ask for your consent.
We keep the information you provide only as long as needed to fulfill the purpose for which it was given and to meet our legal and record-keeping obligations, after which it is deleted or de-identified.

This website is for general information and to help surgeons and patients connect with our facility. Please do not submit personal health information through this website or the application form. Patient health records are collected and handled separately by your surgeon and our facility in accordance with Ontario’s Personal Health Information Protection Act (PHIPA) and other applicable laws.
This website is not directed to children, and we do not knowingly collect personal information from them.
We may update this policy from time to time. When we do, we will revise the date shown at the top of this page.
